What caused the train derailment?

Speeding has been found to be the main cause of the derailment of the Puyuma train in Yilan County, Taiwan that killed 18 people and injured 190 others on Sunday, according to initial findings from investigators this week. The driver of the train, who was seriously injured but released from hospital, is facing a charge of negligence leading to death and has been released on bail. But many questions remain, including how human error with such serious consequences could have been prevented and whether changes need to be made to avoid such preventable accidents from happening again.
