Puyuma train derailment caused by malfunctions, human error: report

A final investigative report on the Puyuma Express train derailment in October 2018 released Monday indicated that the deadly accident which resulted in 18 deaths and nearly 300 injuries was caused by speeding and malfunctions on the train.
